Requirements for In-Facility Training Participation During the Coronavirus Pandemic

 

Arrival Procedures

  • If you are a high-risk student (i.e. a student with asthma, a compromised immune system, diabetes, etc), we recommend that you continue to train via Zoom rather than attend an in-facility session.

  • Do not enter the facility if you exhibit any signs of illness, including cough, fever, shortness of breath, or digestive issues.

  • Use the restroom before arriving at the facility; restrooms will be available for emergency only.

  • Arrive no more than 10 minutes before your scheduled class start time.

  • Enter through the office door only.

  • All students and parents/guardians will be screened for COVID symptoms and their temperature will be checked via thermal scan thermometer. Anyone exhibiting a temperature above 100.4F will be asked to leave and to seek medical advice.

  • While waiting outside for temperature screenings, students and spectators will be required to maintain a minimum of 6 feet distance.

  • All students and parents/guardians who enter the facility will be required to use hand sanitizer upon arrival.

  • All students must be checked in using their member ID badge upon arrival. In the case of a COVID exposure, members will be contacted based on check-in logs.

 

Spectator Guidelines

  • Only one parent/guardian/spectator recommended per child.

  • Spectators should wear a face covering at all times, unless doing so would represent a serious risk to their health or safety.

  • Spectators should maintain a minimum 6 feet of social distance from other families while at the facility.

 

Student Guidelines

  • Students should wear their uniform to class. Changing areas will not be available.

  • Students must follow instructor guidance at all times.

  • Students should maintain a minimum 6 feet of distance between themselves and other students when possible. Avoid high fives, hugs, handshakes, etc.

  • Students over age 5 should wear face coverings when not actively participating in class, unless doing so would represent a serious risk to their health or safety.

 

Exit Procedures

  • Students must sanitize their hands after class.

  • No loitering after class in the building or parking lots.

  • Exit through the viewing area only.

 

Cleaning and Sanitization Procedures

  • Students should wash/sanitize equipment and uniforms after every class.

  • McClendon Martial Arts will sanitize mats and facility equipment frequently.

  • McClendon Martial Arts will frequently sanitize high-touch areas, including door handles and seats.

  • If you have an emergency and need to use the restroom, please alert a McClendon Martial Arts employee so that we may ensure proper sanitization after use.

 

 

 

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

 

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

 

McClendon Martial Arts (MCMA) has put in place preventative measures to reduce the spread of COVID-19; however, MCMA cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending programming at MCMA could increase your risk and your child(ren)’s risk of contracting COVID-19.

 

By signing this agreement, I, the undersigned, acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending MCMA classes and programs, and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

 

I understand that I have the option to attend programs and classes virtually via Zoom in lieu of participating in classes/programs at the MCMA facility.

 

I understand that the risk of becoming exposed to or infected by COVID-19 at MCMA may result from the actions, omissions, or negligence of myself and others, including, but not limited to, MCMA employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at MCMA or participation in MCMA programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless MCMA, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of MCMA, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any MCMA class or program.
